Assessment and Review Team (AaRT)

The newly formed Adult Service has expanded to respond to the needs of adults who

  • have learning disabilities
  • have physical disabilities  
  • are autistic

The Assessment and Review Team (AaRT) support people who are likely to have significant and ongoing need for care and support throughout their lives and are eligible under the Care Act 2014.  This includes young people in transition (14-25) as well as adults 18+. 

Having a disability can have an impact on people’s lives for a number of reasons, including, performing household tasks, managing money, personal care access to the community as well as opportunities for education and employment. 

Our officers will work with the people we support, their families and other professionals to look at their holistic needs and the support required to improve and maintain their well-being.

  • Community Assessment Officers
  • Independent Lifestyle Officers
  • Employment Support Officer
  • Carers Officer

Referral Details

In order to determine eligibility for an adult social care service, we will begin to gather information about you and, where appropriate, will undertake an assessment of your needs, in line with The Care Act 2014 eligibility criteria.

Anyone can make a referral providing they have the consent of the person they are requesting support for or it is in their best interest to be referred.
