Making a referral - School Direct Only

To meet the criteria for a request for consultation, the school needs to provide evidence of at least two completed cycles of the Assess/Plan/Do/Review process. This will show how strategies have been implemented to support the young person and the impact of this. (Exception can be made to this if a pupil has just moved into the area with significant need)

If schools wish to make a referral to Umbrella - Oak Bank Special School Outreach, they can complete the Referral Form which can be found in the Downloads box.

Other notes

Support for Parents

Although Referral can only be accepted from a school, Umbrella staff recognise the importance of working in partnership with parents. Collaboration is key to supporting pupils who are experiencing challenges at school and Umbrella staff aim to work closely with parents and carers when a referral is accepted by the Service.
