Children’s Continuing Care Service

The children’s continuing care team supports families in the community who have children with life limiting or life threatening conditions and complex medical needs. The team aims to enable these children to reach their full potential. Without the support of the team many families would find it impossible to continue to care for their child at home.
The children’s continuing care team is a team of qualified nurses and specialist community nursery nurses. The nursery nurses provide the majority of the care to the children, with emphasis on development and stimulation through play and are clinically trained to meet the identified medical needs of the individual children.
The children’s continuing care team delivers care to children as set out in the individual care plan (EHC), which is written by a Registered Nurse in agreement with parents and multi-agency team. Care provided by the team has an emphasis on normalisation for the children at all times. Play plans are implemented to ensure that children are stimulated appropriately according to their age and stage of development. These plans aim to support each child to fulfil their full potential, and are reviewed as required.
